Organization Participation Agreement
By filling the form below you agree to the following terms and conditions:
  • Duck Race raffle tickets will be sold for $5 each
  • Organizations will earn $2.50 for each ticket sold. The Naugatuck Chamber will make a donation to our organization following the June 3, 2012 event.
  • You are responsible for any stolen, misplaced, or lost tickets.
  • Each ticket stub must be returned in numerical order.
  • No markings should be made on any tickets.
  • Ticket sellers must be 16 or older; ticket stubs must be legible and completely filled out.
  • Tickets cannot be transferred to other organizations.
  • More tickets may be available to us; however; we must sell and return all ticket stubs and money prior to receiving more tickets.
  • Tickets will be distributed on April 12, 2012
  • All unsold tickets must be returned to the Chamber by May 24, 2012.
  • Organizations must return all money collected from the sale of tickets to the Naugatuck Chamber by May 24,2012
